Research reviewed 2026-10-03. Printed edition reviewed: 01/20/25. Reviewed October 3, 2026: edition acceptance depends on surgeon signature date; 01/20/25 is required for signatures July 3, 2025 or later. Check current submission and validity policy.
Select full, partial or recognized prior examination
Ordinary adjustment generally needs a civil surgeon report; consular applicants use the post's panel physician. K/prior-examination exceptions require actual admission, exam date, vaccination and Class A/waiver records.

Record the required report portions
For a recognized partial vaccination-only route, complete Parts 1–5, 7 and 10, with Parts 3/4 only when those helpers are used. Do not require full-exam Parts 6/8 or an unneeded referral Part 9 from every partial-report applicant.
